American Traveler is hiring an experienced RN for a Hospice Case Manager position in a home health and hospice setting, requiring prior hospice case management experience and a VT or Compact RN license.

Details

• Home health and hospice agency setting serving a rural patient population
• Hospice case management role with in-home patient care
• Monday through Friday schedule
• 20-week contract

Requirements

• Active VT or Compact RN license currently in hand
• Current BLS/BCLS certification required
• Minimum 1 year of RN experience required
• Prior hospice case management experience required
• Experience providing hospice care in a home setting required
• Prior experience with rural travel preferred
• McKesson/Netsmart EMR experience preferred

• Candidate should be available for the full duration of the contract with no time-off requests preferred
